The doctors welcome new patients who live within our practice area.
How Do I Register With The Practice?
Due to Covid-19 we are trying to reduce the footfall into the practice. If possible, please open the document on the link below, fill in your answers and email it back to Chgp.firstname.lastname@example.org
One form is needed per person registering.
If you do not have access to emails, you may wish to bring this back to the practice in person, but please do call ahead so that we may give you a quieter time to attend.
Temporary residents are registered in a different way, please contact reception on 01202 772540.
In the same document as the registration form is a health and medical history questionnaire to fill in and you can ask for a health check by the Nurse if requested. This is important as it allows us to get to know you better and collect important information on past-medical health, current problems and medication. It is also an excellent time to ensure that all immunisations, contraceptive requirements, cervical smears and blood pressure measurements are up to date. There can sometimes be a delay in obtaining your records from your previous Doctor and therefore the new patient information is vital to your new Doctor.

Visitors to this country will normally be charged a consultation and prescription fee. Those from certain countries may be entitled to NHS treatment as Temporary Residents. Please ask at reception for details.
