Social Prescribing

Social Prescribing aims to connect people to activities, information, and resources. This is a chance for you to speak to someone in a confidential setting about your life and the things you might be struggling with. An appointment with a social prescriber can provide practical and supportive guidance to figure out what is important to you and find who can help.

What can I help with?

  • Getting involved in local groups, social activities and support groups
  • Accessing befriending services
  • Signposting you to support around housing, benefits, and financial matters
  • Helping with accessing transport options
  • Finding out about volunteering opportunities

Social Prescribing is available through the Canford Heath GP surgery to anyone over the age of 16.

Who can I support?

  •     Low in confidence or self-esteem
  •     Long-term conditions
  •     Socially isolated - having little or no support
  •     Unemployed - out of work or unable to work owing to sickness
  •     Those experiencing bereavement or loss
  •     Carers or ex-carers
  •     People interested in finding new hobbies or interests
  •     People struggling with low mood or anxiety
